Enrollment Requirements

  • Completed Academia Nursing Program Application: A completed application is the first step to creating an enrollment file
  • State Tested Nurse Aid (STNA) Training Program: The STNA Training must have been completed within the past 2 years, or if longer, the applicant must be active on the Ohio Registry
  • Official High School Transcript or GED Certificate (Or Higher Education Completed):
  • Successful completion of the Entrance Exam (TEAS Test): The can be taken at Academia at scheduled times.
  • Essay: An essay between 300-500 words on the following prompt:
    “Describe the characteristics you possess you feel will help contribute to your success in the PN program and in practice as an LPN”
  • Official College Transcripts: Nursing courses are generally nontransferable

Clinical Checklist
Must be completed before first day of clinicals

  • Physical: Must be able to perform physical demands of the program
  • Immunizations:
    • 2 Step Mantoux Tuberculin Skin Test, or as necessary a chest x-ray
    • Hepatitis B series or a signed waiver declining immunization
    • History of 2 MMR Vaccinations or titer showing immunity
    • DTaP Vaccine
  • CPR: Must be CPR certified with the American Heart Association
  • BCI Background Check: Reports that contain excluding documentation may result in the student not being able to attend clinical rotations (FBI background check must also be done if you have lived outside of Ohio more than five years ago)
  • Annual Flu Vaccine: Must be obtained when it becomes available during Flu Season
